Zygofix Ltd. ("Zygofix," "we," "us") is a medical device company registered in Israel (company no. 551565208), located at 17 T'chelet Street, Misgav 2017900, Israel. We develop the zLOCK Facet Fixation System. Zygofix is responsible for the personal information you submit through the patient inquiry form on www.zygofix.com.

2. What information we collect

When you submit an inquiry, we collect: your full name, email address, phone number, country, state, city, whether you are a patient or a family member/caregiver, and the content of your message. Your message may include health-related information, such as details about a spinal condition or procedure. Please share only what is necessary for us to respond.

Our website may also collect basic technical data (such as IP address and browser type) for security and functionality.

3. Why we use your information

We use your information to respond to your inquiry, to provide general information about the zLOCK system and Zygofix, and, if you ask, to help connect you with a physician or medical center.

If your message describes a problem with a Zygofix device, we are legally required as a medical device manufacturer to record and assess it, even though this form is not intended for complaint reporting.

We do not use your inquiry information for marketing unless you separately agree to it.

4. Legal basis and consent

We process your information based on the explicit consent you give when you check the consent box on the form, and, where applicable, to comply with our legal and regulatory obligations. You may withdraw your consent at any time by contacting us. Withdrawal will not affect processing already carried out, or information we must keep by law.

5. Who we share it with

We do not sell your personal information. Inquiries are handled by our team in Israel and our team in the United States, depending on where you live.

e use service providers to operate our website, online forms, and email systems (Squarespace, Zoho, and Microsoft), who process information on our behalf under confidentiality obligations.

We share your information with a physician or medical center only if you ask us to, and with regulatory authorities only where the law requires it.

6. International transfers

Your information may be processed and stored in Israel, the United States, and other countries where our service providers operate. We take reasonable steps to ensure it remains protected wherever it is processed.

7. How long we keep it

We keep your information for as long as needed to respond to your inquiry and maintain our communication with you, and for as long as required or permitted by applicable law, including medical device regulations, which may require certain records to be kept for 15 years or more. When we no longer need your information for these purposes, we will delete or anonymize it.

8. Security

We use reasonable technical and organizational measures to protect your information, including restricted access and encrypted transmission. However, no method of transmission over the internet is completely secure.

9. Your rights

Depending on where you live, you may have the right to access the information we hold about you, correct it, request its deletion, restrict or object to its processing, and withdraw your consent. To exercise these rights, contact info@zygofix.com. We will respond within the time required by applicable law.

You also have the right to complain to your local data protection authority (in Israel, the Privacy Protection Authority).

10. Not for emergencies, medical advice, or device complaints

This form is not for medical emergencies. If you are experiencing a medical emergency, contact your local emergency services immediately.
